Patient Registration

After your family is affiliated with the practice, you may need to add patients (such as a newborn). To do this, visit the registration page.

When adding a new patient, please fill out as much information as possible on the form. At the bottom there is a table of related contacts you may want to connect with the patient. To link one of these contacts to the new patient, check the "Link" box and specify the role this contact has with the new patient.

When finished, click "Submit".
