1. Co-pay policy

As per requirements of all insurance plans we are obligated to collect co-payments at the time of the visit. Please make sure that you have paid your co-pay before leaving the office. If you do not pay at the time of the visit we may charge you a processing fee of $10.

2. Late visits and NO-shows

Our office does everything possible to minimize waiting times. We ask that you arrive five minutes before your scheduled time in order for us to process your visit. To improve scheduling opportunities you must cancel or reschedule your appointment at least 24 hours ahead of your scheduled time. If you come late for your appointment or if you do not cancel in time you may be charged a fee of $25.

3. School and CampForms

If your child is scheduled for a check up we will fill out one school or camp form for free. If you require multiple forms, there will be a $20 fee for each additional form. If you need a school or camp form and your child is not scheduled for a check up that day there will be a $20 fee for each form processed.

4. Referrals

For any new problem your child may have you first need to schedule an appointment to be evaluated by one of our doctors, before seeing a specialist.
For ongoing problems we need to be notified at least 1 week before the appointment to process the referral. Unfortunately, we are unable to process referrals while you wait. Referrals also cannot be faxed. Please check with your sub-specialist office and let us know if the offices already have internet access and therefore have access to the on-line referral system.

5. Insurance Plans Policy

By contracting with insurance companies, we have agreed to file insurance claims for patients who participate in these plans. In order to do so, we must see your child's insurance card at each visit and if you participate with a managed care program, one of our doctors' name must appear on the card. If you do not have the necessary information and we are unable to verify your child's coverage, we must ask for payment at the time of the visit.
