Co-pay Policy

As per requirements of all insurance plans we are obligated to collect co-payments at the time of the visit. Please make sure that you have paid your co-pay before leaving the office. If you do not pay at the time of the visit we may charge you a processing fee of $10.

Late visits and NO-shows

Our office does everything possible to minimize waiting times. We ask that you arrive five minutes before your scheduled time in order for us to process your visit. To improve scheduling opportunities, you must cancel or reschedule your appointment at least 24 hours ahead of your scheduled time. If you come late for your appointment or if you do not cancel in time you may be charged a fee of $75. For initial ADHD evaluations, please notify us at least 48 hours in advance to cancel/reschedule to avoid a $150 fee.

School and Camp Forms

For every well visit, each patient is still entitled to 1 free DOE health examination form and any necessary 504 allergy/asthma/accommodations forms. When not opted into the PSP, additional medical forms (camp/sports forms, school-specific forms, etc.) and written letters (FMLA, work certification, confirmation of diagnosis, etc.) are subject to fees.

Referrals

For any new or existing problem your child may have, you first need to schedule an appointment to be evaluated by one of our doctors, before seeing a specialist. We need to be notified at least 1 week before the specialist appointment to process the referral. Unfortunately, we are unable to process referrals while you wait. We require the specialist name, address, NPI number of the specialist, date of service, CPT codes, phone and fax numbers in order to correctly complete the referral.

Insurance Plans Policy

By contracting with insurance companies, we have agreed to file insurance claims for patients who participate in these plans. In order to do so, we must see your child’s insurance card at each visit and if you participate with a managed care program, one of our doctors’ name must appear on the card. If you do not have the necessary information and we are unable to verify your child’s coverage, we must ask for payment at the time of the visit.

Appointments

We see patients from Monday to Friday by appointment only. Our receptionists answer the phones beginning at 8:00 am Mondays to Fridays, and will be happy to schedule an appointment for your child, usually for the same day if necessary.  If you are unsure about needing an appointment, or have a medical question, our doctors are available 7 days a week to give advice.  We encourage you to call as early in the day as possible, so we can find the most appropriate time for your child. Appointments after 3:00pm are generally reserved for those inevitable, after school emergencies.

Please schedule appointments for any child you want the doctor to see. Often “taking a peek” at a brother or sister who happens to be with you requires time to open the charts and can often lead to delays.

We understand that circumstances may occasionally prevent you from keeping your scheduled appointment. Please notify our office in advance if you are running late or unable to make your appointment. We require at least 24 hour notice to cancel well exams or you may be charged $75. For initial ADHD appointments, we require at least a 48 hour notice to cancel/reschedule or you may be charged $150. Our receptionists will do their best to reschedule you promptly.  We strive to be respectful of your time as well, and will do our best to see that your scheduled appointment is on time.

If your child is sick on a weekend or holiday, please call our office at 718-858-4924 in order to speak with a doctor on call.  Arrangements can be made to have your child seen in our office at the soonest availability.

Sick child visits are generally scheduled on a same day basis. Starting early morning, our phones are answered by our receptionists to make appointments.  When scheduling well child appointments, please plan ahead to allow us to better accommodate your schedule. We make every attempt to be as responsive to your scheduling needs as possible.

Telephone Services

We provide telephone E/M services to our patients. These are non-face-to-face phone call services regarding medical discussions which we submit to your insurance.
